UID Registration

 

  • In order to avail the services offered at the Hospital, the Patient needs to generate a one time UID (Unique Indentification Number).
  • This can be done offline and online as well.
    • Offline UID generation :
      • In order to generate UID offline, you should contact at the main reception of the hospital.
      • UID can also be generated offline , when you visit hospital to avail services on OPD, IPD or Diagnostics or procedures for the first time.
      • A form for the same is to be filled up and handed over at the counter itself.
    • Online UID generation :
      • In order to generate online, you need to download our app, through which you can create an UID
      • A profile set up and an online form is to be filled up and submit through app.
  • A soft copy of of ID Card with life time validity shall be available , which you can take a print out for your future usess.
  • UID will be your permanat number for all your future visits and services in the hospital.
  • The UID generation is done only one time for which you pay very nominal one time charges.
  • The UID generation is done round-the-clock.
  • Please make sure that you provide accurate information in the form.
  • Please carry your UID card for every hospital visit. 
  • Your UID number is important to make sure that you receive timely services and to track your medical records in our hospital.
  • Your UID is also important to avail our discounted and charitable offers time to time.

 


 OPD Services

 

Out Patient Consultation :

  • At Vardaan Hospital, OPD Consultation facility of different departments is conducted on all the week days.
  • In order to avail the OPD consultation services , first you need to book an appointment, either offline or online.
    • Offline appointments
      • can be booked on " Walk in" basis,at Reception counter and an instanat appointment can be fixed depending on the available slot
      • Can be booked telephonically on the given numbers, where our staff shall assist you in smooth scheduling of appointment.
      • You need to pay the applicable OPD Consultation fee on arrival.
    • Online Appointments
      • You may visit our website or
      • book an appointment through our app, which can be downloded from google playstore.
      • You can choose the options to pay the applicable OPD consultation fee by clicking the "pay now" or "pay later" button.
      • In case of "pay now" you are entitled for a special discount on your Consultation fee.
  • In case you are new and visiting the hospital for the first time, you need to generate a UID first.
  • In every subsequent visits you can refer your UID number for easy and smooth processing. 
  • Your appointment request shall be subject to confirmation of availble slot,
    • If the requested slot is available, You shall be provided with an "Appointment confirmation status report" , which you can present at the time of Consultation
    • In case of non vavailibility of the requested slot, you will be asked to choose another suitable options.
  • Once the appointment status is "Confirmed" you shall present the same in the concerned department to avail OPD services
  • Follow Up OPD Consultations :
    • After Primary consultation, your treating Physician may advise follow up consultation visits
    • In majority of instances, for Every subsequent follow up visit the confirmed appointments are pre scheduled at the time of primary visit itself.
    • in case it is not pre confirmed You may need to follow the same process as above
  • Please do not forget to bring all your previous medical treatment, prescriptions and investigation records, if any, during every visit for better services.

 

OPD Special Clinics Consultation :

  • At Vardaan Hospital, OPD special clinics & Consultation facility are availble in different departments on regular basis.
  • For these special clinics , you may refer our website, Apps and social media where notifications are posted on regular basis

OPD Charitable Clinics Consultation :

  • At Vardaan Hospital, OPD Charitable clinics & Consultation facility are availble on daily & regular basis.
  • The financially marginalised and challanged population of socity can avail the services and benefit the sick
  • The mamangement ensures that such charitable care shall be of same standards as for the general population without any descrimination.
  • The Days & timings of currently available charitable clinics are as follows :
    • Obstetrics & Gynaecology : Mon - Sat ( 6 days) : 10.00 am - 11.00 am
    • General Medicine : Mon - Sat ( 6 Days ) : (10.00 am - 11.00 am)
    • General Surgery : ( Mon - Sat ) ( 6 Days) : 10.00 am - 11.00 am)
  • For these Charitable clinics , you need to pay only a nominal amount of 20 Rs, as the registartion.

  • In These Charitable clinics investigations are availble on subsidized charges 

 

Virtual Care ( Consultation) ( E visits and Vedio visits )

  • With Vardaan Hospital Virtual care our patients can access the same Consultation services from our specialists as they are used to from the comfort and convenience of home.
  • Currently, There are following three types of virtual consultation facility are available  :
    • Online Chat/ or text consultation
    • On line Audio consultation
    • On line Vedio Consultations
  • These E Consultations are available on pre appointment scheduling and pre payment baisis, for non emergency care or care that does not require an in person physical examination
  • To fix an appointment for E Consultations you need to subscribe the Patients portal or our app from the play store and apply or request an online consultation service.
  • On Confirmation of your appointment , you need to pre pay the applicable consultation for the e-consultation services
  • On line consultation services are availble only for clinical issues related to patient's sickness. these services are not to discuss administrative and financial issues related to treatment cost etc.
  • Inquiries For administrative and financial services can be resolved by contacting our reception counter and Hospital back office services.

 


IPD Services & Admissions : (In Patient Care)

 

General Guidelines for all Admissions

  • On your arrival for admission, you need to approach our "Admission Cell"  located at the main reception in ground floor. 
  • Our admission staff is available to answer any of your queries and guide you to choose a suitable beds and rooms.
  • All admissions are under the sole discretion of the authorised Hospital Consultants and their "ADMISSION ORDERS".
  • All room confirmations are subjected to the availability, on the date of admission.
  • However, in planed admissions, you may pre-book and reserve the rooms of your choice. You may Consult admission cell for assistence.
  • We have different class of beds and rooms available and you may consult admission cell for details. 
  • In case of Emergency, you may be admitted in emergency ward first and later may be shifted to rooms of your choice or ICU depending on the patient's condition.
  • You are required to fill up an "ADMISSION REQUEST FORM" and give all your demographic details, category, entitlements and autherisations, insurance, payment terms and conditions.
  • You need to make sure that you clarify all your querries and doubts about charges before admission.
  • Should you have any questions regardig billing before and during your stay , our Billing staff would be always there for assistance.
  • At the time of admission you need to pay an initial deposit, which is applicable for all patients except Patients under Government schemes of cashless facilty (i.e. RSBY or other such)
  • Any change in beds or rooms category ( upgradation to higher or to a lower category) is permissible and is subject to availibilty. You need to place a request at the nursing station. Our admission cell shall be there for assistance. 

Guidelines During your stay as admitted patients 

  • Medications :
    • In campus Pharmacy services are availble round the clock in the ground floor and is currently out sourced to the third party. 
    • All medications prescribed can be purchased from the Pharmacy counter by paying cash to the paharmacy counter.
    • Pharmacy services counter shall remain open from 8.00 am to 9.00 pm everyday.
    • In case of emergency, however, the pharmacy shall open and provide medicines in the night hours also.
    • In case of non availibility of medicines Patients attendents are allowed to bring it from outside medical stores.
    • The unused medicines of patients at the time of discharge can be returned at the pharmacy before discharge.
    • No return of medicines shall be enetertained once the patient is discharged from the hospital.
    • Patients with cashless/credit facility ( Corporates, TPA / Insurance/ Govt schemes) are provided medicines from in house hospital pharmacy and are advised not to take madicines on there own
  • Procedures & Cross Consultations:
    • Procedures  planned or emergency, if any, shall be performed and are under sole descretion of the treating consultant.
    • Patient and attendents are advised to discuss and understand the details of plan of treatment from the treating consultants.
    • Your treating physician may advise battery of investigations and cross referrals which he feels neccessary, before considering any procedure.
    • Your patient may be shifted to Crital care unit before and after the procedure depending on his condition 
    • A time to time conselling and discussion with your treating physician is advised.

 

  • Housekeeping :
    • Housekeeping team maintains hyigene and cleanliness and provides bedside assistance to the patients.
    • Rooms and Toilet cleaning is done on regular daily basis
    • Patients bed making and patients linen are taken care of by the ward nursing staff on regular basis.
    • Our house keeping supervisor / nursing incharge / floor incharge are on daily rounds of the wards to take care of proper cleanliness and hyiegene.
    • In case , should you require any house keeping assistance or have any complaints related to this you may ask our supervisors or floor incharge.
    • You may directly approach our center Head in case Your issues remail unresolved.

 

  • Fooding and Meals services :
    • The Hospital provides a three time complimentary well balanced vegetarian diet for all the patients.
    • The daily diet includes breakfast, lunch and dinner on regular and fixed time basis.
    • The diet depends on the medical condition and is based on the instructions of the treating physician.
    • Please consult your treating physician before bringing any outside food.
    • Meals are available for Attendents and relatives on subsidised charges per meal.
    • In order to avail meals services for Attendents and relatives they should place the order well in advance before 9.00 am at the main counter or nursing station.
    • Attendents meals order Once placed and arrived , can not be considered for cancellation. 

 

  • Facility management services :
    • In case of any faults in electricity,TV, cables, Air conditioning or plumbing and sanitation etc please notify the ward nurse or floor incharge on duty for assistance.
    • Please do not attempt to shift or repair any electrical , TV or plumbing problems by yourself.
    • Do not attempt to touch or  manipulate Central Oxygen line, knobs, patient monitors, ventlators or any medical device.
    • Should you find any problem, please inform the nursing station ASAP..
    • The entire premises is under CCTV survellience in the best interest and safety of the patients, hence do not attempt to move or enetr to restricetd zones.
    • Lift is for the patients purpose hence avaid unneccessary usage of lift.
    • Usage of Mobile phones near patients is advised to be restricted. 

 

  • Extra Facilities and services :
    • Considering your and your patient's overall smooth and uneventful stay hospital provides certain extra facilities
    • These facilities are not the part of the main treatment of Patient but many a times are essential for over all comfort of patient and attendents.
    • These services fetch a nominal subsidised charges and is to be paid in cash.
    • A List of Extra facilities with charges is availble at the main reception counter which you can refer in order to avail those services 

 

Guidelines of Discharge :

  •  the discharge process is initiated only after the patient’s consultant deems him/her fit for discharge.
  •  Routine Discharge timing is before 12.00 p.m.
  •  Patients vacating the room between 12.00 p.m. - 4.00 p.m., on the completion of all discharge formalities would be charged 50% of the room tariff.
  •  Patients vacating the room after 4.30 p.m., on completion of all discharge formalities would be charged a one day room tariff.
  •  Patients are advised to collect their discharge summary before leaving the hospital.
  •  Patients requiring the charge slip details for reimbursement purposes can request for the same at the time of discharge.
  •  In case a patient does not wish to continue his/her stay at the hospital, he/she can, in consultation with his doctor, ask for the DOR ( Discharge on request) or Discharged Against Medical   Advice (DAMA)
  •  Before leaving, the patient needs to check his/her room carefully for any personal belongings.
  • To avail of an ambulance/transport facility, the patient can put in a request at the nursing counter/reception counter.
  • Please Note: - Discharge process may take 3 hours- 4 hours to be completed from the time the treating consultant initiates the discharge. There may be more delay in case of final authorization is required from TPA for insurance.

 


 Payment Guidelines on admission

 

Cash Payment Facilty :

Planned Admissions with Cash Payments:

  • Financial arrangements for your hospital stay and procedures must be made prior to or at the time of admission.
  • The Bed  and room charges and a rate list card is available at the counter.
  • The estimated costs in case of planned surgeries are provided at the estimation counter of the Inpatient Services Department.
  • An initial deposit at the time of admission in accordance with the room category is to be deposited on admission. 
  • Charges vary as per room category, irrespective of the medical services.
  • Cash, paytm, googlepay and credit / debit card payments are accepted.
  • Cheques are not accepted for the payments and deposits.

 

Emergency Admissions with Cash payments :

  • Emergency charges are applicable on surgery charges on Sunday, holiday for whole days & on other days from 9:00 pm to 8.00 am.
  • Emergency charges will be 15% extra on all consultation, investigation & procedure in OPD/ A & E/ Day-care & also 15% extra on surgery amount.
  • No emergency charges are applicable for planned surgery on Sunday & Holiday for already admitted patient.
  •  Any shortage of initial deposit during emergency admissions has to be cleared within 12 hours of admission or before 12.00 noon of the next working day.

 

Intial Deposit on admission :

  •  All patients are required to pay an initial deposit at the time of admission as specified by the admission counter.
  •  For surgeries or procedures the entire cost is to be deposited atleast 12 hours before the due surgery/ procedure.
  •  In case of packaged Treatment, the entire cost of the package has to be deposited at the time of admission.
  •  All The deposits would be adjusted against the final bill.
  •  Charges for investigation, operation theatre charges, and the doctor’s fees are subjected to change in accordance to the particular room category chosen.

 

Interim Deposits during stay and admissions :

  • Interim bills would be produced on alternate days and will be available at the IP billing counter.
  • Patients are required to make a deposit within 24 hour of receipt of the interim bills.
  • If the bill is not cleared within 24 hour then all the credit facilities, whether pharmacy or any other will be discontinued.

 

Full and Final Settlement at the time of Discharge :

  • Full and Final Bill will be generated , once the treating physician discharges the patient.
  • Patients are required to settle all dues before leaving the hospital.
  • Any advances or previous deposits shall be adjusted against the bill and balance is to be paid or refunded as paer the billed amount.
  • In case of any descripency in the bill it needs to be resolved before discharge or before leaving the hospital.
  • No claim shall be entertained once the patient is dischrged and leaves the hospital.

 

Refund

  • In case of cancellation of IPD admission within one hour , the entire amount shall be refunded after deducting a nominal administrative Charges.
  • However, In case once the patient is settled in the bed or room for more than an Hour, a one day charges shall be applicable on refund
  • In case the amount has to be refunded on the same day as that of the deposit, then the refund would be made by cash.
  • In case the amount has to be refunded on the next day of the deposit, then the refund would be made by cheque. I
  • In such a case, the bearer of identity proof and authority letter from the patient /relative would be entitled to collect the balance.

 

Treatment Finance with Easy EMIs

 

 

 

Credit Facility :

  •  Credit facility is offered only to the patient from Corporate & Insurance and Patients under Govt schemes carrying valid Cards.
  •  TPA’s having a tie up with Vardaan Hospital for patient covered under Medical Insurance, it is mandatory to get approval for cashless facility from TPA before patient gets admitted.
  •  However if the patient gets admitted in emergency, then this approval for cashless facility must be produced before the discharge.
  •  Hospital can not be responsible ,In case there is a delay in authorization, patient authorization or denial of authorization from the insurance/TPA and 100% of estimated cost or the difference cost to be deposited at the time of admission or before the surgery.

 

Corporate Admissions with cashless facility :

  • Corporate clients are required to submit their company’s credit letter of entitlement clearly accepting the responsibility to pay the charges of your medical treatment and procedures
  • In case of planned admissions " credit letter of authority" is to be submitted on the date of admission.
  • In case of emergency admissions, the credit letter has to be submitted within 12 hrs. of  admission.
  • In the case of the non-submission of the company’s credit letter, the patient is deemed as a cash patient and an initial deposit is to be deposited.
  • The same would be refunded (at the time of   discharge) against the submission of the company’s credit letter.
  • Patients shall be allocated the rooms as per their entitled category mentioned in the authority letter.
  • Charges for investigations, operation theatre and the procedures, doctor's fee shall be applicable as per agreed terms and conditions in MOU.
  • Opting for a room/bed category higher than their entitled category are required to pay the difference in charges arising thereafter.
  • Charges for investigations, operation theatre   charges, and the doctor’s fees are subjected to change in accordance with the particular room category chosen.

 

Admissions under TPA & Insurance with Cashless facility

  • If you have a Health insurance policy, you need to present the TPA and insurance details at the time of admission.
  • In Planned admissions a Pre approval is requierd to be obtained from the TPA
  • In case of emergency, Approvalcan be obtained within 24 hours.

 

 Admissions under Governent Schemes ( RSBY & Others) with Cashless facility

  • Patients carrying a valid card with their names shall be entitled for cahsless treatment facilty under the govt scahemes in our hospital.
  • The card holder should have an adequate balance in the card for the specified treatment.
  • Some of the common surgeries and procedures are not specified and are not available for the private hospitals 
  • In such cases , patient will not be entitled for the cashless facility in the hospital.

 Payments for Extra Facility :

  • Attendent's Extra Bed/ Extra meals/ pilow/ linen / mattress shall not be the part of Medical bill
  • Extra toiletories / Mosquito repelents / Laundry services / juices/ cold drinks /waater bottles/ Magzines/ etc and
  • Expenditure which are not the part of the Treatment of Patient shall not be a part of the patients medical bill
  • These all shall be paid in cash, for which a seperate bill shall be raised and issued 
  • This shall be applicable for all patients irrespective of Cash / cashless and credit facility

 

 

Attender & Visitors Guide :

  • Attender's and Visitor's pass is meant for the safety and security of your patient and valuables.
  • Each Patient will be given a cpmlimentory  "Attender's Pass" and a "Visitor's Pass"  at the time of admission.
  • Attendent is the person who must accupy the patient for 24 Hours through out the stay, with a valid attender's pass. 
  • Attender's pass is mandatory to access various departments in the hospital.
  • Without a valid Attender's pass , an attendent shall not be permitted to stay with the patient
  • VISITOR is a person who is allowed to visit the patient during visitng Hours, with a valid visitor's Pass.
  • Only one visitor per patient will be allowed at a time.
  • Visiting Hours are regulated to ensure That your admitted patient gets proper rest, which is essential for speedy recovery.
  • Restrictions in the number of visitors ensures prevention and reduction in infection rates, thereby ensuring patient's safety.
  • Children younger than 12 years are not permitted as attender or visitors due to safety reasons,without special authorization.
  • Visiting Hours shall be as follows :
    • Morning : 7.30 am - 8.30 am ( 1 Hour)
    • Afternoon : 11.30 am to 1.30 pm (2 Hours)
    • Evening : 5.30 pm - 8.30 pm ( 3 Hours) 
  • These passes are meant for Hospital use only, please do not carry it outside the hospital.
  • These passes shall be returned at the reception at the time of discharge.
  • The validity of these passes expires automatically once the patient is discharged from the hospital
  • A visitor shall not be permitted to visit the patient without a valid visitors pass.
  • If the visitor needs a visitor pass , he/she may call the patient's attendent to come down and present the visitor pass
  • Attender and visitor passes are free for the first time.
  • If lost, a fresh attender / visitor pass shall be issued only after payment of 100/ foreach apss. 
  • *Attender's and visitor policy is subject to cahnge under special conditions like COVID, epidemics & pandemics etc and further restriction may be in place for the safety of patients
